Letters of Guardianship must be submitted to ETF and approved before a guardian can act on behalf of a member.

If Letters of Guardianship are amended to change guardians or grant additional authorities, the new Letters of Guardianship must be submitted to ETF. ETF will follow the most recent Letters of Guardianship that we have on file and approved.

To end a guardianship on file with ETF, an Order on Petition for Termination of Guardianship must be submitted to ETF.

Letters of Guardianship may be rejected if:

  • They are Letters of Guardianship of the Person.
    • ETF can only accept Letters of Guardianship of the Person to provide health insurance or apply for benefits as a beneficiary for a person under 18 years old.
  • Pages are missing or unreadable.
  • Not signed by the court.
  • The document is incomplete.

What to Submit

Submit the Letters of Guardianship, or Order on Petition for Termination of Guardianship, including any supporting documentation, to ETF. On the document or a separate page submitted at the same time, provide the member’s ETF ID, if possible.

If you are submitting Letters of Guardianship from a state other than Wisconsin, you may also need to submit the order of guardianship/conservatorship.

How to Submit

Submit the document to ETF using one of the following methods:

  • Mail, fax, or drop box
  • Online with Box
    • The link above will take you to the Box website, which is a secure and confidential way to send your document to ETF. You don’t need an account with Box to submit your document. Basic contact information is required in case the document is not readable.
    • After you submit, you'll see "Success! Your file has been submitted."
    • It will take a few days to be uploaded into your records, at which point ETF staff will be able to view your document. If you have any questions, please contact ETF.

Timeline

It may take a couple of weeks for the guardianship documents to be processed and initially reviewed by ETF. This timeline may increase if you are submitting documents from a state other than Wisconsin.

We encourage you to file Letters of Guardianship with ETF as soon as possible. A guardian cannot take any actions permitted by the document until it is approved by ETF.
