Mutual of Omaha Long-Term Care Insurance Administered by HealthChoice

Mutual of Omaha offers a long-term care insurance plan through HealthChoice. 

Long-term care includes medical services, such as nursing care or therapies. It also includes supportive services, such as bathing, dressing, getting in and out of bed, taking medicines, or preparing meals. Long-term care can be provided in a variety of settings, including nursing homes, your home, an adult day care center, or a group living arrangement with supportive services.

Enrollment

HealthChoice offers enrollment into the Mutual of Omaha Long-Term Care Program throughout the plan year.

The Long-Term Care Insurance Program is available to state employees, retirees, their spouses, and parents of employees, retirees, and spouses who reside in Wisconsin.

Coverage acceptance is not guaranteed. Applicants will be subject to medical underwriting. Some illnesses or pre-existing conditions may make an applicant ineligible for coverage. However, once the policy is issued, coverage will not be limited or exclude any pre-existing conditions or illnesses.

  • Employers do not contribute to the premiums for this plan.
  • Universities of Wisconsin and UW Hospital and Clinics employees may have different supplemental plans available. Check with your human resources/benefits office for details and instructions. Universities of Wisconsin employees may refer to Universities of Wisconsin Employee Benefits page.
