Wisconsin law prohibits ETF from receiving copies of vital records, including digital copies. ETF has created a new form that employers must fill out when receiving vital records.
Effective immediately, employers must send ETF the Employer Attestation for Documentation Received (ET-1908) form to verify that they have personally viewed their employees’ original required documents to verify benefit coverage eligibility. Employers should not send digital copies of vital records.
The form includes fields for names and ETF IDs for employers, employees, and dependents, as well as possible vital records, including:
- Birth certificates
- Divorce decrees
- Marriage certificates
- Death certificates
